Prenatal Visits
We offer complimentary “meet the doctor” visits for all our expecting parents to come visit our office and make an informed choice regarding their soon to be family addition. These visits must be scheduled in advance and take place at noon Monday thru Friday either in person or via phone. We recommend trying to schedule your visits at least 4 weeks prior to your due date. If you would like to request a specific physician please do so when scheduling your visit.

Well Child Visits
We perform regular well child visits during the following schedule:
1 Week, 1 Month, 2 Months, 3 Months, 4 Months, 5 Months, 6 Months, 9 Months, 12 Months, 15 Months, 18 Months, 2 Years, 2 1/2 Years, 3 Years, Once per year thereafter
Sick Visits
Sick visits may be scheduled during regular office hours Monday thru Satuday. We can accommodate a limited number of same day sick visits but an appointment MUST be scheduled ahead of time. As soon as you know you need to bring your child in, please call the office to schedule an appointment. If your primary physician is unavailable we will offer you one of our other providers. For more chronic issues an appointment may be offered on a different day.
We are NOT able to accommadate WALK-IN patients. AN APPOINTMENT MUST BE MADE prior to your child's visit.
Telemedicine Visits
We also are able to provide Telemedicine Visits for certain types of complaints or needs. These appointments need to be scheduled in advance with our office in the same manner as in-person visits. Please contact the office to determine if a Telemedicine visit is appropraite and to schedule the appointment with one of our providers.
Pre-Op Physicals
Pre-op physicals are performed 24-48 hours prior to the scheduled surgery date. As soon as you have the surgery scheduled please call to schedule the pre-op physical. In the event the surgery is scheduled on a Monday, the pre-op physical will be scheduled for the Friday prior. If there are any specific forms that need to be filled out please have them with you at the time of the scheduled pre-op physical.
Vaccination Only Visits
Vaccine only visits with one of our medical assistants are performed BY APPOINTMENT ONLY Monday thru Saturdays. Please arrive promptly and check-in iva text message. We highly recommend calling the day prior to confirm that the vaccine needed is available (particularly with the seasonal flu vaccine).
Ear Piercing
Ear piercing is typically performed during your child’s well child visits but is also available at 8am or 1pm Monday thru Friday. The cost is $60 and includes the first set of earrings.
School Forms, Athletic Forms & Vaccination Records
If you need to order school forms, athletic forms or vaccination records, you can now order them online by clicking here. We are no longer accepting requests by email or phone. Forms ordered online can be emailed to you or will be available for pick-up at any of our office locations within 2 to 5 business days from the date of request depending on the delivery speed selected. There is a charge of $10 per form with the exception of vaccination records which are free of charge. There is a $5 fee for any rush orders.
